Urban Health Plan’s Sapphire Ball promises to be the highlight of 2019! Celebrating our 45th anniversary and the 90th birthday of our founder, Dr. Richard Izquierdo, the gala will highlight the accomplishments of UHP and honor individuals who have made our work possible. The Sapphire Ball is our largest fundraiser of the year. Proceeds from the gala will go towards the Foundation for Urban Health and Well Being (which supports under funded programs within Urban Health Plan and other health focused initiatives).

Urban Health Plan was founded in 1974 by Dr. Richard Izquierdo, a local physician, to create access to health care for the residents of the South Bronx. Today, it is a health system comprised of twelve community health centers, one mental health center, twelve school-based health centers and three part-time clinics that serve residents of the South Bronx, Corona, Queens and Central Harlem. In 2018, Urban Health Plan served more than 87,000.
